When you click the "Submit" button, you will be directed to a Thank You page which acknowledges the form has been sent. Simultaneously, email notifications are sent to you and select Club Officers.

The email you will receive includes the Club's "Waiver of Liability". All you have to do is print it, sign it and include it with your check to Ed Silberhorn. It's as simple as that.
